Waivers
Club Sports Waiver

All Club Sport athletes must read and digitally sign the Club Sports Waiver each academic year to participate. If athletes participate in more than one Club Sport team, they must complete a separate waiver for each sport. The waiver serves as a roster for the club, and if a waiver is not completed, athletes may not participate.

Participants under the age of 18 must download the hardcopy PDF of the waiver to be signed by a legal parent or guardian. The completed waiver is to be turned into the Campus Recreation Office or sent to [email protected].

Event Registration

All Club Sport events home and away must be registered via the Club Sport Event Registration Form 30 days in advance.

Club Travel Rules and Regulations

All travel by any Club Sport at the George Washington University must be approved by the Office of Campus Recreation 30 days in advance. Each club is solely responsible for its mode of transportation to and from club activities. Clubs are permitted to rent 12-passenger vans, minivans, and cars for travel. Students must be 20 years of age and approved student drivers for rentals. The university also recommends that Club Sports utilize charter motor coach service with professional drivers when applicable.

Driver Authorization
  • All drivers must be formally authorized to drive themselves or others around for club-related activities.
  • Authorizations are valid for one year from the date of authorization.
  • Students must be 20 years or older to be authorized to rent and use regular vehicles. That includes cars, minivans, and pickup trucks.
  • Students must be 21 years or older to be authorized to rent 10-12 passenger vans.
  • Enterprise is the only rental car company authorized for use by GW student orgs.
  • Each authorized driver is approved to drive a maximum of 250 miles at a time. Traveling in excess of 250 miles requires a change of drivers. If an organization is driving in excess of 500 miles round trip, an overnight stay is required.

Driver Authorization Steps:

  1. Complete the Annual Student Driver Authorization Application in its entirety.
  2. Send to current advisor Erik Strouse, Associate Director of Programs, at [email protected] along with a picture of your current, valid driver's license.
  3. Once signed by Erik Strouse, upload completed authorization form to the GW Club Sports Internal Driver Application Form google form.
  4. Complete the GW Driver Safety Training & Test that will be provided directly by the GW Club Sports Internal Driver Application Form immediately after submitting your information.
  5. Send test results to [email protected] and [email protected]

GW Fleet 10-12 Passenger Van

  1. Student must be 21 years or older.
  2. Complete all steps for the regular driver authorization.
  3. Complete the VAN safety test provided by the GW Club Sports Internal Driver Application Form.
  4. Complete the 10 - 12 Passenger Van Safety Acknowledgement in order to drive a larger capacity can and email to [email protected] and [email protected]
  5. Submit a Motor Vehicle Request to GW Fleet to get access to Fleet Commander. You will be contacted to complete a road test with GW Fleet.
  6. After successfully passing your road test, you will receive an email back from Fleet Commander authorizing you to reserve vehicles at their website.
How to Request Rental Cars/Hotels/Flights

Requirements for Travel Requests:

  1. All travel requests must be submitted 30 days prior to planned travel date.
  2. Engage accounts must have enough revenue to fund the entirety of the trip at the time of submission.
  3. Must have current authorized drivers. Drivers may not be just starting the authorization process at the time of submission because it could take up to three weeks to be cleared.
  4. Must fill out the Club Sports Event Registration Form for the event being requested. 
  5. Must complete the Anthony Travel Request Form for each requested event. 
  6. Must follow the Request Submission Steps below.

Request Submission Steps:

  1. Fill out the Club Sports Event Registration Form.
  2. Fill out the Anthony Travel Request Form.
  3. Send the completed Anthony Travel Request Form to [email protected] for verification of submission requirement criteria.
  4. Forms not meeting requirement criteria will be sent back to the teams for modifications.
  5. Forms meeting requirement criteria will be sent to Anthony Travel by the Campus Recreation team with club team members attached. Anthony Travel will take over communicating with the teams to finalize travel plans.
Athletic Trainer/Injury Resources

Sports Medicine Services

The Office of Campus Recreation and DSA in collaboration with the George Washington University Hospital is committed to providing high quality medical services to student-athletes currently participating in Club Sports at GW. The Club Sports Medicine staff consists of certified Athletic Trainers providing services including event coverage, injury evaluation and management.

How to Make an Appointment

The Sports Medicine Department offers appointments when a club sports athlete is injured during practices or games. If you are injured during a scheduled club practice or game (home or away), please follow these steps to be seen by an Athletic Trainer for an initial assessment:

  1. Please complete and submit the online Injury Report Form within 24 hours of the injury occurring.

Requesting an Athletic Trainer for Scheduled Events

High-risk sports will need to request an Athletic Trainer to be present for their home events. When completing the Club Sports Event Registration Form you will select needing one. Campus Recreation will reach out to the Athletic Training team to schedule.

Pre-participation Screening

It is highly recommended and encouraged to have an annual orthopedic and general physical examination prior to participating in any sporting or physical activity. The George Washington University Student Health Center provides complete pre-participation physical examinations.

Insurance

It is strongly recommended that all Club Sport student-athletes have an Accidental/Medical insurance plan in effect prior to participating in Club Sport activities. For more information about insurance, see the information provided through the Student Health Center.

Concussions

Club Sports Medicine Staff

Chris Duncan, ATC​

 

Chris Duncan has been an Assistant Athletic Trainer at GW since 2006. He recently moved over to the GW Hospital and will work with our club teams this season.  A native of Asheville, N.C., Duncan earned his Bachelor's Degree in Athletic Training from Erskine College in 2002 and his Master's of Education in Health and Human Performance from Western Carolina University in 2004. Prior to coming to GW, Duncan served as an Assistant Athletic Trainer at the University of Maryland working with the Baseball team from 2004-06. His clinical interests include concussions, knee and shoulder rehabilitation, and technological advances in Athletic Training. He resides in Alexandria, VA with his wife, Marla and son, Christopher, Jr.

Finances

No Authority to Bind

You must submit requests in Engage for all expenditures. Note that you do not have authority to bind GW funds - please ask goods/services providers for non-binding price quotes only and never sign a contract/agreement/document or place an order outright.

Making Purchases

  1. Ask vendor for non-binding price quote or contract
  2. Schedule a time here to meet with OrgHelp team member, Allison Sherwood, to discuss next best steps.
  3. If approved for purchase and after determining which account or accounts you will use to process your transaction needs, submit a purchase request in Engage.
  4. If a credit card payment is needed, Allison will help you process the transaction in the moment.
  5. If the purchase requires a contract, please review and complete the Contract Information Sheet and send to Erik Strouse at [email protected]. If the vendor also provided a contract, please send that along as well. If a formal contract must be drafted, Erik Strouse will work on that with the vendor. Reminder: students are not authorized to sign contracts.

CSC Additional Funds Requests

All additional funding requests for club sports will be considered by the Club Sports Council. The finance committee will meet every Wednesday evening to discuss requests and allocate funds. All requests must be submitted by 8 p.m. on the Tuesday prior to the Wednesday meeting day you would like to be considered.
